Home Based/Family Case Management

Family Outreach Center's Home Based / Family Case Management Program is designed to address the counseling needs of the children and adolescents with severe behavorial and emotional dysfunctions. Referral of children and their families that fit this protocol are made to Family Outreach Center by Network 180.  The children referred may also be court or delinquent wards who are involved with the 17th District Court - Family Division, under a legal guardanship with a relative, in kinship care, or residental services.

Nuclear families, kinship care families, and single heads of households involved in the treatment experience are most likely impacted by:

  • Mental illness
  • A lack of understanding of child development/child management
  • Substance abuse
  • Inadequate nurturing, training, and child rearing themselves
  • Isolation and extreme lack of emotional support

Objective:

Reduce the number of children and adolescents with severe behavorial and emotional problems entering residential treatment or psychiatric hospitals.

Eligibility:

Children, adolescents, and families residing in Kent County.

Funding:

Funding is provided by Network 180, the Heart of West Michigan United Way, and the genereosity of corporate donors and private individuals.
